Fitness Waiver

If you are coming along to our Zumba® or Zumba® Gold classes for the first time, I will get you to complete an information form and a fitness waiver (please note this is not required for Aqua Zumba® classes as these classes are governed by the pool).

I will have these available for you to complete at your first Zumba® or Zumba® Gold class, or you can download the form below and complete it before you arrive.

Photography

Please note I don't allow photographs or filming during classes.  

On rare occasions, I will take a photograph before or after a class, but if you prefer not to be photographed then there is no requirement to sign this section of the waiver form.  I will always announce if I am taking a photo and ask for anyone to step aside if they prefer not to be in it, no reason or explanation required.  I want everyone to feel comfortable at all times and this includes the use of photographs :)
